PLDT Home Rewards x Caltex Rewards: Fuel Your Fiber Speed

1. Promo runs from September 25 to December 31, 2024 only.
2. To be eligible to the promo, customer must meet the following criteria:
   • Must be a bonafide PLDT Home Rewards member as of December 31, 2024 in accordance with the definitions set by the program mechanics and terms & conditions which are found: https://pldthome.com/loyalty-and-rewards. 
   • Must not be an employee of PLDT, Inc., Smart Inc., Chevron Philippines Inc., PLDT and Chevron Philippines Inc. advertising agencies, PLDT-related parties, affiliates and their relatives up to the second degree of consanguinity or affinity, and any other party who are directly involved in organizing promoting or conducting the promo and draw.
3. PLDT Home Rewards are entitled to the following:
   • Free P150 worth of Caltex Rewards Points, this can be activated during their first gas-up with Caltex. To redeem this reward, they will have to:
     1. Sign-in to their PLDT Home Rewards Portal
     2. Go to REDEEM, then select Caltex to redeem the code: CR- PLDTHR2”
     3. Download the CaltexGo App, then apply the redeemed code “CR- PLDTHR2” during registration in the referral code field. Members should use the same number that they used for PLDT Home Rewards when they register to the Caltex Rewards program for easier verification.
     4. Activate the reward by using the CaltexGo app during the member’s first gas-up for a minimum spend of P1,000 in Caltex stations nationwide. A total of 15,000 Caltex Rewards points equivalent to P150.00 will be credited to the new member’s CaltexGo app.
     5. PLDT Home Members with existing Caltex Rewards account or membership will not be eligible to redeem the P150 worth of Caltex Rewards points.
   • Grand Raffle Draw wherein there will be one (1) winner of P100,000 worth of Caltex Rewards points, five (5) winners of P10,000 worth of Caltex Rewards Points and five (5) winners of P10,000 Maya cash credits. To qualify and earn raffle entries, they will have to:
     1. Active member of Caltex Rewards with at least one fuel transaction in any Caltex stations during the promo period;
     AND
     2. Payment of monthly PLDT Home Bill during the promo period, every monthly payment will enable the Member to earn ONE (1) RAFFLE ENTRY per month.

     Note: Hence, a PLDT Home Rewards Member can earn a maximum of THREE (3) RAFFLE ENTRIES during the promo duration.

4. Program members may check their valid raffle entries by logging in to their PLDT Home Rewards account at https://rewards.pldthome.com

RAFFLE DRAW

5. Customer can only win once. In case a customer is drawn more than once during the raffle draw, the customer will get the prize of higher value.
6. Cut-off registration and submission of entries will be on:
   • December 31, 2024, 11:59 PM
7. Winners will be randomly picked via electronic raffle draw on:
   • January 14, 2025
   • Winners will be notified and announced on January 21, 2025
8. The draw will be held at the PLDT Office 5/F Ramon Cojuangco Bldg., Makati Avenue, Makati City with the presence of DTI Representative/s and Caltex Rewards Representatives for verification purposes.
9. During the draw, there will be a total of eleven (11) winners who will win the following prizes:
   • One (1) winner of the major prize – P100,000 worth of Caltex Rewards points.
     1. Five (5) winners of Php 10,000.00 Maya cash credits.
10. Five Winners (5) winners of Php 10,000.00 worth of Caltex Rewards points.

PLDT Home Rewards will share the winners’ full name and mobile number to Maya and Caltex Rewards for the crediting of prizes purposes only.

11. Each winner will be verified by both PLDT Home Rewards and Caltex Rewards during the raffle draw. Participating members don't need to submit their addresses, as we already have records for these members with PLDT Home Rewards membership.

CLAIMING AND NOTIFICATION

12. Winners will be notified via call out, registered mail, email and text message from PLDT Home Rewards.
13. To claim the prizes, winners must present the following documents:
    • Registered mail notification received as winner of the promo.
    • Two (2) Government issued Identification Card (IDs)

    Failure to present above-mentioned documents shall be grounds for disqualification

14. In case the winner will send a representative to claim his/her prize, the following documents should be submitted, in addition to the documents being required from the winner:
    • Notarized Special Power of Attorney (SPA)
    • Representative’s two (2) Government issued Identification Card (Ids)
15. All documents provided by the winner will be verified by PLDT, Inc.
16. Prizes should be claimed within sixty (60) days from the receipt of registered mail. Otherwise, it shall be forfeited in favor of PLDT, Inc. and Chevron Philippines Inc. with prior to DTI approval.
17. Prizes worth in excess of Php 10,000.00 and above subject to 20% withholding tax to be shouldered by PLDT.
18. Other related expenses relative to receiving the prizes shall be shouldered by the winner.
19. Prizes are not convertible to cash and are non-transferable.
20. By participating in this promo, customers agree to be bound by the official rules and regulations and decisions of PLDT, Inc. Participating in this promo also constitutes permission of the winners to participate in any advertising purpose or activity in line with the promotion including the publication and use of their name at PLDT’s website and official social media platforms.
21. Participants who shall be found violating any of the Rules and Regulations of this promo shall be disqualified from joining in this promo.
22. By accepting the prizes, winners agree to hold PLDT, Inc., Chevron Philippines and its employees free and harmless from any loss, injury, or damage caused by or arising from their participation in the program or caused by/or arising from their acceptance and/or use of the prizes.
23. Acceptance of the prizes constitutes permission to use the winner’s name and photograph for advertising purposes without further notice or compensation, unless prohibited by law.
24. By virtue of an entry to the promo, the eligible participants signify their absolute and unconditional acceptance and agreement to all the Terms & Conditions stipulated therein.
25. PLDT, Inc. reserves the right to verify the eligibility of the winning entry. PLDT, Inc. reserves the right to disqualify registered customers who are suspected of abuse of the promo system or found to be in breach or violation of Rules, Terms and Conditions of this contest.
26. The promo organizer’s decision with regard to the promo mechanics, with concurrence of DTI, is final.
27. Full mechanics, which include details about how to join, claiming of prizes and raffle date will be posted on the PLDT Website and official social media pages.
28. PLDT Home Rewards will share the list of winners to DTI after the raffle draw.

CONTROL MEASURE

29. A random electronic raffle draw will be conducted to ensure fairness in determination and announcement of winners.
